MEMBERSHIP TERM:
•    The initial membership shall be for a period of three months. A member shall not be entitled to terminate or suspend his/her/them membership during the initial period. If the contract is terminated before this term, there will be an early termination fee applied to the credit card on file at a cost of $100 and no other program benefits can be utilized from that date onward.
•    The member can cancel the membership at any time after a three-month period with a two week written notice to admin@ymmcosmetic.com.
AUTOMATIC PAYMENT AGREEMENT:
•    Monthly membership payments will be auto charged every month to the member’s designated credit/debit card on file.
•    The member is responsible update any changes to your credit/debit card information, for example an expiry date change or credit card cancellation. This can be done through the Raklet app. If there is not a valid credit/debit card on file, we reserve the right to suspend the membership until the payment has been received. If no payment has been received after 30 days we reserve the right to cancel the membership and if the member wishes to enroll again they will have to pay the $25 administration fee and all outstanding balances.
•    YMM Cosmetic and Laser Clinic reserves the right to review enrolments periodically and make any changes to the membership. It is our promise that we will give the member a 30-day notice of any such changes that may occur. (Example: Membership fee changes or change in services and discounts under the membership benefits)
SCHEDULING POLICY
•    There is a minimum of 24-hour cancellation policy. If you NO SHOW, we will notify you of this, your first no show will not be charged; however, YMM Cosmetic & Laser Clinic reserves the right to charge a NO SHOW FEE of $50 to be paid before the next appointment booking. This payment will be put toward your treatment unless you fail to attend the appointment without notification, then the $50 fee goes directly to the service provider you were scheduled with.
You are responsible for scheduling your treatment appointments, YMM Cosmetic & Laser Clinic is not responsible for service rollovers. We recommend you schedule your next treatment while you are at the clinic.
TERMINATION/SUSPENSION OF MEMBERSHIP:
•    YMM Cosmetic & Laser Clinic reserves the right at anytime to cancel or suspend memberships in the event of the following:
•    The member has monies due to YMM Cosmetic & Laser Clinic that remain unpaid for more than 30 days after the due date. This includes the membership fee itself.
•    The member knowingly provides false details when applying for the membership.
•    If YMM Cosmetic & Laser Clinic terminates the contract for any of the above reasons, they reserve the right to retain any monies received during the membership agreement to cover the costs incurred as a result.
TERMS AND CONDITIONS:
•    YMM Cosmetic & Laser Clinic reserves the right to vary, add or eliminate any of the services provided in the program from time to time. As noted above, a 30-day notice will be provided to members in any such instance.
•    YMM Cosmetic & Laser Clinic reserves the right to close or modify clinic hours with or without notice or to cancel client appointments with reasonable cause.
•    The MEMBERSHIP promotions CANNOT be combined with any other promotional gifts such as gift cards or promotional offer/sales. (Example: Black Friday Deal)
•    Neuromodulator treatment units cannot be utilized on friends and family nor can the units be accumulated. (Example: If you have not used your units for 2 consecutive 3 month intervals, you cannot accumulate to 120 units)

This membership agreement between YMM Cosmetic & Laser Clinic and the member shall be effective as of the date of the application approval date. Complimentary Neuromodulator Treatment will commence 3 months after the application approval date, the member will be responsible for booking your treatments on and every 3 months thereafter, we will allow a 3 day grace period before date only. 

I HEREBY AGREE TO THE MEMBERSHIP TERMS AS STATED THROUGHOUT THE CONTRACT
